Midwest Academy Victim Statement Information

THE FORM MUST BE SUBMITTED BY JANUARY 22ND TO BE CONSIDERED BY THE COURT.

To fill out a Victim Statement to the court, follow these instructions:

1. Download the PDF form here.
2. Either print the form and fill it out manually, then re-scan it, or use an app that allows you to write on PDFs, such as Adobe Fill and Sign.
3. Fill out form as completely as possible.
